Achilles tendon pain is one of the most common injuries we see in active aduts, particularly those who take part in sports involving repeated sprinting, jumping and sudden changes of direction. While it often starts as a minor niggle, ignoring the warning signs can lead to a more persistent and frustrating injury.
A recent patient presented with mid-portion achilles tendinopathy. Following a course of Shockwave therapy alongside a structured load management and strengthening exercise programme, they made an excellent recovery with a successful and gradual return to sport.
When a Small ‘Niggle’ Becomes a Bigger problem
The patient first noticed a mild ache in their achilles tendon after playing tennis. It wasn’t enough to stop them from playing, so they continued with another session. The following morning, however, the pain had increased significantly.
Like many people with achilles tendinopathy, the pain was at its worst during the first few steps after getting out of bed. The discomfort became severe enough that they had to rest from sport. Noticeable thickening and swelling of the achilles tendon developed, along with tenderness with touching.
In the clinic during the assessment, we also identified reduced strength in the affected leg, particularly during push-off movements, which are vital for the explosive lunges and quick changes of direction required in tennis.
Understanding Achilles Tendinopathy
Tendinopathy develops when the tendon is repeatedly exposed to more load than it can successfully recover from. Over time, the tendon becomes painful, stiff and less able to cope with the demands of activity.
Complete rest may reduce symptoms temporarily, but it rarely restores the tendon’s capacity to tolerate sport. Instead, successful treatment focuses on gradually rebuilding strength while carefully managing the amount of load placed through the tendon.
Why We Chose Shockwave Therapy
Shockwave therapy delivers high-energy audio waves into the affected tendon. Studies show it can reduce pain, stimulate tissue healing and promote tendon remodelling at a cellular level. Peer reviewed evidence supports its use as an effective adjunct to exercise in patients with persistent achilles tendon pain. A randomised controlled trial revealed that patients who use shockwave therapy alongside a structured loading exercise programme, experienced greater improvements in pain and function than those performing exercise alone. What We Did
Alongside shockwave therapy, we introduced a progressive calf strengthening programme to gradually increase the tendon’s ability to tolerate load. Rehabilitation progressed from controlled strengthening programme exercises to heavier resistance work, single leg calf-raises, hopping drills/plyometrics and finally to sports specific movements designed to replicate the demands of tennis.
Careful load management was equally important as Shockwave. Rather than complete rest, activity was modified so the tendon received enough stimulus (the load) to adapt without aggravating it and creating flare-ups. THis approach is supported by recent tendon research, which consistently identifies progressive loading as the foundation of successful achilles tendinopathy rehabilitation.
The Result
Over the following weeks, the patient’s morning stiffness gradually settles, tenderness reduced and confidence in the affected leg improved.
Strength testing showed clear improvements in the calf function and push off power returned as the progressed through the rehabilitation programme.
Once they could tolerate sport-specific loading without pain, they gradually returned to tennis. They have since resumed playing without pain or restrictions and have remained symptoms free.
