It’s often described as a “silent condition” due to it developing over many years without symptoms, until a minor fall or sudden movement results in a fracture. Understanding what osteoporosis is, how it develops and how lifestyle changes can protect your bones is the first step toward living confidently and maintaining independence and quality of life.
What is Osteoporosis?
Osteoporosis is a condition that causes bones to become weaker and less dense. Most people with osteoporosis would describe their bones as ‘thin, brittle, and / or fragile’. The term osteoporosis means “porous bone”, making these individuals more prone to fractures. Common fracture sites include the spine, hips, wrists, and ribs, and these fractures can occur from minimal trauma, such as a simple fall, bending or coughing in severe cases. Bone is not a static tissue, it is dynamic, and constantly adapting to the demands placed upon it. Osteoporosis develops when this adaptive process becomes imbalanced.
Bone Remodelling Explained
Healthy bone is maintained through a process called bone remodelling, which involves two main cell types:
- Osteoclasts – break down old or damaged bone often referred to as bone resorption
- Osteoblasts – build new bone often referred to as bone formation
In younger adults, these processes are balanced. As bone is broken down, it is replaced with new bone of equal strength. Peak bone mass is usually reached in early adulthood around late 20s to early 30s.
What Goes Wrong in Osteoporosis?
With ageing, particularly after menopause in women, the balance of bone remodelling shifts:
- Osteoclast activity increases or remains the same
- Osteoblast activity slows down
The altered balance of bone remodelling results in net bone loss. During the imbalance of bone remodelling, bone becomes thinner at the outer surface and less connected internally. Therefore the bones begin to lose their structural integrity, leading to a reduction in bone density and a reduced capacity to absorb load.

Despite osteoporosis being commonly associated with ageing and hormonal changes, other factors such as low body weight, smoking, limited physical activity, or certain medications can also increase risk.
Hormones and Other Influences
- Oestrogen plays a crucial role in stopping bone breakdown. After menopause, reduced oestrogen levels accelerate bone resorption.
- Calcium and vitamin D deficiency impact bone mineralisation.
- Inflammation, inactivity, smoking, excess alcohol, and long-term steroid use further disrupt bone remodelling.
- Mechanical loading such as movement and weight-bearing activities are essential for signalling osteoblasts to build bone. With reduced mechanical loading osteoblast signalling is reduced resulting in bones weakening.

You often hear of osteopaths and physiotherapists explaining movement is medicine, but with osteoporosis, the right type of exercise matters. Research has shown a combination of weight-bearing, resistance, and balance exercises can help stimulate osteoblasts for bone remodelling, strengthening and maintenance of strength in muscles, and improve stability and coordination to reduce risks of falls and fractures.
- Weight-bearing exercises: Walking, dancing, swimming or low-impact aerobics help stimulate bone growth.
- Resistance training: Using free weights or resistance bands strengthens muscles and bones, particularly in the spine and hips.
- Balance and flexibility exercises: Activities such as Pilates or gentle yoga improve stability, reducing the risk of falls.

Nutrition: Feed Your Bones
Diet plays a key role in bone health due to bone being a mineral-rich tissue that depends on adequate nutrition.

- Calcium: Essential for bone structure. Found in dairy products, leafy greens, fortified plant milks, and almonds.
- Vitamin D: Helps the body absorb calcium. Sunlight exposure, oily fish, and eggs are good sources. Supplements may also be necessary, especially in winter.
- Protein: Essential for bone matrix and muscle strength. Aim to include a source of protein at each meal. This can include various types of meat, fish, eggs, nuts and tofu.
- Magnesium, Vitamin K, and Zinc: Support bone mineralisation and metabolism. You can opt to take supplements to ensure you are having the correct intake.
- Avoid excess alcohol and caffeine: Both can interfere with calcium absorption and bone metabolism.
Overall, it is advised to ensure you are having regular, balanced meals ensuring an adequate calorific intake. Caffeine, alcohol and smoking should be stopped and/ or limited as much as possible.
Falls Prevention: Protecting What You Have
Since fractures often occur after a fall, fall prevention is as important as strengthening bones. Key strategies include:
- Check your vision and hearing regularly as changes to these senses can negatively affect balance.
- Keep your home safeby removing clutter, secure rugs, install grab rails, and ensure good lighting.
- Wear supportive footwear.
- Improve your posture and core stability to aid steadiness and confidence. This can be achieved with regular exercise and targeted balance training that is safe for your bone health.
- Medication Review from your GP as some medications can cause dizziness or low blood pressure increasing your risk of a fall.
